A bra-line back lift is a surgical procedure that removes excess skin and fat from the upper and middle back to create a smoother, firmer contour. The name comes from the location of the incision, which is placed discreetly along the bra line so that any scar can be concealed under clothing or swimwear. This procedure is particularly effective for addressing folds, bulges, or laxity in the back area that have developed due to ageing, significant weight loss, or genetic factors.

Procedure Overview
This procedure is suitable for people with loose or sagging skin and stubborn fat in the upper or mid-back region that does not respond to diet or exercise. It is particularly beneficial for women who notice back rolls or bulges around the bra area, as well as for men with similar concerns in this region. Ideal candidates are at or near their stable ideal weight, in good overall health, and have realistic expectations about surgery and scarring.
Aim
The aim is to tighten and smooth the upper back by removing excess skin and fat, restoring a more streamlined contour and toned appearance.

CONSULTATION
In your consultation, the surgeon will examine the distribution of skin and fat in your upper and mid-back, assess your skin elasticity, and review your medical history to ensure you are fit for surgery. You will discuss your goals and learn about the specific surgical technique, incision placement, and the type of result you can expect. The surgeon will explain how the scar will be positioned to align with your bra line or natural skin folds for maximum concealment. You will also receive information on recovery timeframes, possible risks, and ways to optimise healing, such as maintaining a healthy weight and avoiding smoking. Any combination with other body contouring procedures, such as an arm lift or lower body lift, can also be discussed.
PREOP PREPARATION
The general ways you can prepare for any surgery will include eating a healthy diet which includes all the macronutrients important for daily living. It will help to avoid ultraprocessed foods, and those foods high in refined sugars such as cakes, biscuits or chocolates.
It is important to avoid smoking (including passive smoking), vaping for at least 6 weeks in the run up to your surgery as this can decrease blood flow to the skin and make the healing process slower. Similarly please avoid recreational drugs as these can interfere with having an anaesthetic.
Finally preparation of the body area to be worked on is equally important. For back lift procedures we would recommend working hard on over arm mobility such as being able to rasie the arms high above the head. Post operatively this movement will feel tight and difficult and it will be easier to restore a full range of motion should you be strong at this before hand. Similarly muscle strengthening exercises remain important too – and incorporating a 6 week program of lateral pull downs / pull ups / and horizontal rows into a regular gym regime will be of great benefit moving forwards.
SURGERY
The bra-line back lift is performed under general anaesthetic. The surgeon makes an incision across the upper or mid-back, positioned so that it will be hidden under a bra or along a natural crease. Excess skin is removed, underlying tissues are tightened, and any stubborn fat pockets may be reduced using liposuction. The incision is then closed with sutures, and dressings or compression garments are applied to support healing and help the skin settle smoothly over the back contour. If combined with an arm lift or mastopexy, the scars are seamlessly connected.
RECOVERY
Swelling, bruising, and mild discomfort are common in the first few days and can be managed with pain relief medication. You will need to wear a compression garment for several weeks to help control swelling and support the newly shaped tissues. Most patients return to light activities within 1–2 weeks, but strenuous exercise and lifting should be avoided for around 4–6 weeks. Follow-up appointments will be scheduled to monitor healing and ensure that the scar is settling well.
Frequently Asked Questions
Is bra-line back lift surgery safe?
When performed by an experienced surgeon in an accredited facility, a bra-line back lift is generally safe. As with all surgery, there are potential risks, including infection, bleeding, wound healing problems, fluid accumulation, and scarring. These risks can be minimised by following all pre- and post-operative care instructions and attending follow-up appointments.
Will the scar be visible?
Yes, but the incision is positioned so that it is hidden by a bra or swimwear and often blends into the natural skin crease over time. Proper scar care and protection from sun exposure can further improve its appearance.
How long do results last?
Results are typically long-lasting, especially if you maintain a stable weight and healthy lifestyle. Natural ageing will still cause gradual changes to skin and tissue over time.
When can I resume normal activities?
Light activities can usually be resumed within 1–2 weeks. Strenuous exercise or heavy lifting should be avoided for 4–6 weeks or until your surgeon confirms it is safe.
Can it be combined with other surgeries?
A bra-line back lift is often combined with procedures such as an arm lift, lower body lift, or breast surgery to achieve a more comprehensive body contouring result.
